There are situations where we are required by law to disclose information without your consent, including: when there is a serious and imminent threat to your safety or the safety of others; when abuse or neglect of a child, elder, or vulnerable adult is suspected; or when required by a court order or other legal process.
This website and your information
This website does not collect protected health information. The contact form on this site is not a secure clinical communication channel — please do not submit diagnoses, treatment details, or other sensitive health information through it. Secure communication with your therapist occurs through HIPAA-compliant channels established at the start of your care.
Changes to this notice
We reserve the right to change this notice at any time. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future. We will post a copy of the current notice on this website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with A Restored Hope or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
